A notary public is a person of required honesty, credibility, truthfulness and integrity appointed by the state government to serve as an impartial witness in performing a variety of acts related to the signing of important documents, taking oaths and affirmations and performing other acts authorized by law. We are public servants and must serve all members of the public equally.
I can notarize a variety of documents, including affidavits, power of attorney, wills, deeds, and more.
You will need to provide a valid government-issued photo ID, such as a driver's license or passport, to be notarized.
Unfortunately, I'm not able to provide advice or instructions on what notarial act is needed. You'll have to contact the issuer of the form for clarification.
That is called a subscribing witness and can only be done for documents requiring an acknowledgement. You as the subscribing witness will have to take an oath and sign the document as well. Also, you'll have to bring one credible witness and it has to be someone the notary knows.
Unfortunately, the law prohibits us from notarizing a document that contains blanks or is known from our experience to be incomplete.
